Female, 20s

Goals: This young woman in her 20s wanted to reshape her nose to create better balance with the rest of her face. She was bothered by a dorsal hump and wanted a softer slope to the nasal bridge, along with a slightly lifted and more defined nasal tip. Her goal was a refined, natural-looking nose that complemented her facial features rather than changing her overall appearance.

Procedure: A closed rhinoplasty was performed to reduce the dorsal hump and create a softer profile. Endonasal osteotomies were used to narrow the bony bridge. The nasal tip was refined with a conservative cephalic trim and dome-binding sutures. A vertical strut and bilateral alar rim grafts were added to provide structural support and maintain the position and shape of the nasal tip.

Outcome: She is very happy with her new nose and feels that it achieves the changes she wanted while fitting naturally with the rest of her face. The softer bridge, narrower nasal bones, and more refined and slightly elevated tip create a balanced result without making her look like a different person.

Key Principle: A beautiful rhinoplasty result should not only look good today but also remain stable over time. Refining or reducing the nose can weaken its natural support if that support is not preserved or reconstructed. In this patient, the vertical strut and bilateral rim grafts provided additional structural support to help maintain the refined tip position and nasal contour for a stable, long-lasting result.
