Goals
This patient in her 20s presented with a poorly defined, wide nasal tip and broad alar base. She wanted a smaller, more refined nose that would look cute and feminine while remaining natural. With her large, beautiful eyes and well-shaped but smaller lips, her nose was drawing attention away from her other facial features. Our goal was to create a nose that would better balance and complement her face.
Procedure
I performed a rhinoplasty with dorsal reduction, tip narrowing, increased tip projection and rotation, and alar base reduction. Tip refinement included double-dome binding sutures with lateral crural steal, along with spreader grafts, a tip strut, and bilateral alar rim grafts to provide shape and structural support.
Outcome
Her new nose is smaller, narrower, and more defined, with improved tip shape and rotation and a narrower alar base. More importantly, her nose now fits naturally within her face. Her beautiful eyes stand out, and the relationship between her nose and lips is more proportionate. She is very happy with her balanced, feminine result.
Key Principle
Facial proportions matter more than absolute size. A successful rhinoplasty is not simply about making the nose smaller, it is about shaping the nose so that it complements the eyes, lips, and overall facial structure, allowing the patient’s best features to stand out.
