This gentleman presented in his 60s seeking both functional and cosmetic rhinoplasty. His goals were to improve his nasal breathing while creating a softer, more refined appearance, without changing his identity or making his nose look unnatural.
After carefully discussing his goals, we proceeded with an open rhinoplasty with septal reconstruction. The procedure included conservative dorsal reduction along with spreader grafts to support the middle vault and nasal airway and alar rim grafts to provide additional support to the nasal base and nostril margins.
The result is a softer, more balanced nose that remains natural and appropriate for his face. Just as importantly, his nasal function was addressed as part of the same operation, and he now reports excellent breathing.
This case demonstrates an important principle in rhinoplasty: improving appearance should not come at the expense of structural support or nasal function.
