Lower Blepharoplasty – Female in Her 30s

Goals
This patient was bothered by early lower-eyelid aging, including excess skin, crepiness, and prominent tear troughs. The irregular contour made the area difficult to camouflage with makeup. She wanted smoother, more youthful lower eyelids while still looking completely natural.

Procedure
I performed a lower blepharoplasty through a subciliary incision, combining conservative skin excision with fat transposition, septal reset, and canthopexy. The goal was to smooth the transition between the lower eyelid and cheek while maintaining appropriate support of the lower eyelid.

Outcome
Her lower eyelids are now smoother, with resolution of the tear-trough deformity and improvement in skin crepiness and excess. Makeup is easier to apply, and the result looks refreshed and natural rather than surgically altered.

Key Principle
Successful lower blepharoplasty requires a careful balance between repositioning fat and removing skin. The goal is to create a smooth lower eyelid and lid-cheek transition without over-removing tissue or creating a “done” appearance.
