Goals: Following a previous nasal fracture, she developed a deviated bridge, dorsal hump, tip asymmetry, and a small amount of hanging tip, along with difficulty breathing. She wanted a nose with beautiful, smooth lines and natural flow that fit her facial features—straight, feminine, and completely unoperated in appearance, with a good nasal airway.
Procedure: Closed functional and cosmetic rhinoplasty with dorsal reduction, medial and lateral osteotomies to straighten and narrow the bony pyramid, septal correction, and reconstruction of the internal nasal valve. A left spreader graft was used to improve dorsal alignment and support the internal valve. A septal strut graft and bilateral alar rim grafts provided tip and alar support while allowing controlled narrowing and a subtle elevation of the tip.
Outcome: A straighter, softer, and more feminine nose with clean dorsal lines and improved facial balance. The tip is narrower and subtly elevated without appearing over-rotated, while maintaining natural alar contours. Her nasal airway was also improved.
Key Principle: Natural feminine rhinoplasty is about proportion, not simply making the nose smaller. Precise control of tip rotation with structural support helps prevent an overly rotated or operated appearance, while preserving enough dorsal height creates a smooth, elegant profile that flows naturally with the face. Tip rotation and structural support are closely related, and excessive rotation is a classic feature of an overdone rhinoplasty.
