One of the most common questions I hear from patients who have lost a significant amount of weight on medications like Ozempic®, Wegovy®, Mounjaro®, or Zepbound® is:

“What’s the best treatment for Ozempic face?”

The answer surprises many people.

There isn’t one best treatment.

In my practice, I use a variety of treatments for Ozempic face, including fillers, collagen stimulators, fat grafting, and facelift surgery. The key is understanding that all of these treatments work, they simply solve different problems at different stages of your weight-loss journey.

Choosing the right treatment isn’t about picking the newest procedure or the one you’ve read the most about online. It’s about matching the treatment to your anatomy, your degree of facial aging, and, perhaps most importantly, where you are in your weight-loss journey.

The Most Important Principle: Timing

When I evaluate someone with Ozempic face, the first question I ask isn’t, “Do you need a facelift?”

Instead, I ask:

“Are you still losing weight?”

That question changes everything.

If you are actively losing weight, your face is still changing. As additional facial fat is lost, your appearance will continue to evolve. Because of that, I generally recommend thinking about treatment in two phases:

  • Temporary treatments while you are still losing weight.
  • Definitive treatments once your weight has stabilized.

This approach allows you to look your best throughout the process while avoiding performing permanent procedures before your face has finished changing.

While You’re Still Losing Weight: Temporary Treatments

Many patients don’t want to wait six months or a year before addressing the changes they see in the mirror—and they don’t have to.

If you are still actively losing weight but would like to restore a healthier, more youthful appearance, temporary treatments can be an excellent solution.

In my practice, these include:

  • Hyaluronic acid fillers
  • Radiesse® (calcium hydroxyapatite)
  • Sculptra®
  • Other collagen-stimulating biostimulatory injectables

Each works a little differently.

Hyaluronic acid fillers provide immediate volume and are highly customizable. They are particularly useful because they can be adjusted as your face continues to change.

Radiesse provides both volume and collagen stimulation, making it useful for patients who have mild skin laxity in addition to volume loss.

Sculptra works differently. Rather than acting as a traditional filler, it stimulates your body to produce its own collagen gradually over time, creating a subtle and natural improvement in facial fullness.

These treatments allow patients to maintain a refreshed appearance while their weight-loss journey continues. Since they are temporary, they can easily be modified as your facial contours evolve.

Once Your Weight Has Stabilized: Definitive Treatment

Once you’ve reached your goal weight and maintained that weight for several months, your facial changes become much more predictable.

This is the time to discuss definitive treatment.

At this point, I often recommend surgical rejuvenation for patients who have developed significant skin laxity, facial descent, and volume loss.

The most effective long-term treatment is frequently a facelift combined with fat grafting.

Why I Often Combine a Facelift with Fat Grafting

One of the biggest misconceptions I see is the belief that a facelift restores facial volume.

It doesn’t.

A facelift is designed to reposition descended facial tissues and tighten the deeper supporting structures of the face. It beautifully restores the jawline, neck, and cheeks, but it does not replace the fat that has disappeared with weight loss.

Fat grafting addresses that missing piece.

Using your own fat, I can restore volume to areas that have become hollow, such as the cheeks, temples, under-eye region, and around the mouth. Because this is your own living tissue, the results are natural in both appearance and feel.

These two procedures complement one another perfectly.

The facelift restores youthful facial position.

Fat grafting restores youthful facial volume.

When performed together, they produce a result that neither procedure can achieve alone.

Every Patient Is Different

Some patients have primarily lost volume but still have excellent skin elasticity. They may do beautifully with injectable treatments alone.

Others have developed loose skin, jowls, and neck laxity in addition to volume loss. In those patients, fillers alone may add volume but cannot lift tissues that have descended. They often achieve a much better and more natural result with a facelift combined with fat grafting.

The treatment plan should always be individualized.

 

My Philosophy

One of the things I tell patients is that Ozempic face is not a complication of weight-loss medications—it’s often a sign of successful weight loss. Unfortunately, the face tends to lose fat just as readily as the rest of the body.

Fortunately, we have excellent ways to restore youthful facial contours.

The key is choosing the right treatment at the right time.

If you’re still losing weight, temporary treatments such as hyaluronic acid fillers, Radiesse, Sculptra, and other collagen stimulators can help you continue your weight-loss journey while maintaining a natural, refreshed appearance.

Once your weight has stabilized, more definitive procedures such as a facelift combined with fat grafting can provide long-lasting rejuvenation by addressing both skin laxity and lost facial volume.

There is no single treatment that is right for everyone.

The best treatment is the one that matches your anatomy, your goals, and the stage of your weight-loss journey. That’s why every consultation in my practice begins with understanding not just how your face looks today, but where you are headed tomorrow.
